Sure I've read that that now-infamous scene wasn't noticed by the film's producers at the time? It was a genuine 'blooper' that shouldn't have happened.
Once it was highlighted by fans, the decision was taken not just to leave it in the film, but also add the 'dink' sound effect when head connected with door...
